To begin the adoption process for a puppy from Good Day Doodles, please fill out, sign and return this form.  Upon approval you will be asked to provide a deposit of $500 which will be applied towards the total purchase price of $2,500.00.  This deposit is non-refundable for any reason other than the event that a puppy cannot be provided to you.  The balance of $2,000.00 (not including shipping if applicable) will be paid when the puppy is 6 weeks old (approximately two weeks prior to the puppy leaving Good Day Doodles).

We will be happy to provide shipping for your new puppy.  If you require shipping please let us know in advance so that we can make proper arrangements.  Typically, the cost is around $300.00.  Good Day Doodles will determine the safest means possible for the arrival of your new puppy!
